What Does Denial Code CO-58 Mean?
Denial code CO-58 means the service billed is not covered under the patient's specific plan. Unlike CO-50 (not medically necessary), CO-58 is a coverage determination — the insurer is saying this service is excluded from your benefits regardless of medical necessity.
When CO-58 Denials Can Be Challenged
The exclusion is not clearly stated in your plan documents
Coverage exclusions must be explicitly stated in your Summary Plan Description or Evidence of Coverage. If the specific service is not clearly excluded, you have grounds to appeal.
The service falls under a different covered category
Sometimes a service is denied under one code but would be covered under adifferent billing code that more accurately describes the service.
Mental health parity applies
If the service is mental health or substance use related, the Mental Health Parity Act may prohibit the exclusion if comparable physical health services are covered.
The exclusion violates state law
